Remove INCLUDED_FCNTL.
* xterm.c (INCLUDED_FCNTL):
* callproc.c (INCLUDED_FCNTL):
* alloc.c (INCLUDED_FCNTL):
* systty.h (INCLUDED_FCNTL): Remove all uses, not needed anymore.
(emacs_get_tty, emacs_set_tty): Declare unconditionally.
